I authorize Swann Beauty Aesthetics to debit the credit card account indicated in this web form for the noted amount on the schedule indicated. This payment is for Aesthetic treatments and products. This may be as (i)
pre agreed instalments for single treatment following initial deposit (ii)pre agreed instalments for course of treatments to be spread over 12 months prescribed for my individual needs and concerns (iii)pre agreed subscription to saving plan designed to help manage cost of treatments of my choice.

I understand that refunds are at the discretion of Swann Beauty Aesthetics. All complaints should be brought to management as per complaints policy. Swann Beauty accepts that there can be unforeseen circumstances which are unavoidable, but cancellations are expected to be made at least 24hours in advance if possible, or a cancellation fee of £10 will added to treatment balance (at the discretion of Swann Beauty).

I understand that this authorization will remain in effect until the schedule end date, or until I cancel it in writing, which ever comes first, and I agree to notify the business in writing of any changes in my account information or termination of this authorization at least 15 days prior to the next billing date. I have the right to cancel this agreement at anytime, at cost of £20, but must ensure that all treatments performed are paid in full. I can expect any overpayments accrued as part of annual treatment plan or saving plan subscription to be returned in full within 7 working days (minus £20 cancellation fee).

If the above noted payment date falls on a weekend or holiday, I understand that the payment may be executed on the next business day. I certify that I am an authorized user of this debit card and that I will not dispute the payment with my credit card company, so long as the transaction corresponds to the terms indicated in this web form. I will ensure that funds are available for payment at agreed time, and understand that Swann Beauty will not be responsible for any costs incurred for declined payments, and accept that these will be added to my final balance.
